This registration is for an Affiliate Call-up Registration only.
Eligibility:
A player registering as an Affiliate can play a maximum of ten (10) games. If the team wishes to make the player a full-time player he must register as an NCDC Player and complete the NCDC Player registration at that time.
This registration is required to be completed before you take the ice with an organization.
This registration session will allow you to complete contact information, sign up for participation, and submit payment. Please read through each page carefully to ensure that all information is provided accurately. To complete registration through our secure site, please have your Visa, MasterCard, Discover, or checking account information available.
This $100 Registration Fee is non-refundable.
All players in the USPHL must sign insurance disclaimer, waiver and release of liability agreement in order to register in the USPHL.
Players and Parents acknowledge and understand that the organization and league have strongly recommended that they obtain insurance coverage for injuries and or losses arising from the hockey activities and, if they elect not to obtain or maintain such insurance, the decision not to do so is their voluntary and knowing act. Further acknowledge and understand that in the event they sustain an injury of any kind, they will be fully responsible for any costs or expenses incurred to treat said injury and agree that they will not seek reimbursement from the organization or league and agree not to commence any action in a court of law to compel the organization or the league to pay for same.
All players 17 or older by December 31, 2024 will be required to go through background screening and mysport safety training.
You will complete your my sport training in the registration link that will be emailed to you.
That registration link will include a charge for $2 that will be added to your checkout to pay abuse prevention systems for abuse awareness training as required by the safe sport act of 2017. NCSI charge for background screening is $20.
